The village permit comes before the pour
The Village of Liverpool Code Enforcement office requires a permit to pave or modify any driveway or parking area. That covers a full replacement and a change to the drive you have now. Call the office with your address and say exactly what the contractor plans: tear out, widen, repour or resurface.

Patch, resurface or tear out
Scaling usually traces to too little entrained air, low strength, a wrong mix, heavy de-icer on young concrete, poor finishing or short curing. A resurfacing coat treats the symptom, and any repair is only as strong as the layer beneath it. Tell the contractor to remove the unsound layer, not cover it. A patch will also show: repair material does not match the color of the original.

Ask for a written scope that names the mix, the air content and where each joint goes. If a contractor will not write it down, that tells you something too.
A driveway with a few hairline cracks and a sound surface may need nothing at all. Ask for that answer if it is true.
Give each slab room to move
Isolation joints keep a driveway from pushing against the garage slab, the steps and the walk, so each piece can move on its own and cracking drops. Look at the edge where your drive meets those pieces. If a new pour will butt against them with no gap, tell the contractor you want the joint in the scope and on the drawing.
Then ask when the pour will happen. Cold nights in the weeks after a pour are what the first winter is about, so a date matters as much as a price. Price the job against that date and book it when you are satisfied.
